Who can request a Good Faith Estimate
You are entitled to a Good Faith Estimate if you are:

• Self pay
• Choosing not to submit services to insurance
You may request a Good Faith Estimate before scheduling services or at any time during care.
What the estimate includes
Your Good Faith Estimate may include:
• Expected charges for visits or services
• Testing or evaluations that are reasonably anticipated
• Related care that is part of your treatment plan
If your care plan changes, we will discuss updated expectations with you.
Your rights if costs are higher than expected
If you receive a bill that is substantially higher than your Good Faith Estimate, you may have the right to dispute the charges.
